I've found that when you create a new goal and improvement actions, the new actions may not appear on the team dashboard.
Just manually refresh the dashboard instead of doing a "back" action. You can do this by clicking on the hamburger, click on "my teams", and click on your team again. This will take you to the dashboard and refresh the list.
